Registration Requirements:

(all documents must be the Originals)

• Valid Florida Licenses – RN, LPN, CNA
• Certifies – HHA
• Florida Driver License or ID with photograph
• Proof of Automobile Insurance
• Proof of eligibility to work in the US (e.g.) work permit, permanent resident card, US birth certificate or passport, social security card or driver license
• A medical physical within the last 6 months of registration, signed by a medical Doctor (MD) or Osteopathic Physician and must state that you are free of communicable diseases.
• Level 2 background screen
• Liability Insurance
• A minimum of 3 references ( all references are checked)

 

Continuing Education Courses(CEU)

. Domestic Violence – 2 hours (all caregivers)

. Alzheimer’s – 2 hours (all caregivers)

. Assisting with Medication – 2 hours for 40hour HHA

. Assisting with Medication – 3-4 hours for 75 hours HHA and CNA

. A Valid CPR card (must be approved by the America Heart Association).
